Lost or Stolen Tabs

Lost, stolen, or damaged tabs can be replaced by any registered owner on record

• An Affidavit of Loss form can be completed in advance with a notarized signature or the form can be completed in our office with us witnessing the signature.
• The fee of $13.75 ($19.75 for travel trailers) will replace the tab and print a new registration reflecting the updated tab number.

If your tabs were lost in the mail…

If you didn’t receive the vehicle tabs you purchased, contact the licensing office you purchased them from. Typically, you can replace a tab at no cost, if you meet these requirements:

• We verify that we issued the tabs, but you haven’t received them within 15–90 days.
• You had your current mailing address listed on the vehicle record at the time of purchase.
