Replace a Lost Title
A Replacement Title can be purchased in person or by mail using the following information below.
In Person:
- All registered owners must be present to declare the original title lost, or any absent owners must provide a notarized affidavit of loss and a notarized title application to be included in the processing documents.
- If the current record shows a Legal owner, the Affidavit of Loss must be provided by the Lender. The Replacement Title will be mailed only to the legal owner.
- The replacement is $35.50.
By Mail:
- A letter of request signed and dated; indicating a request for a Replacement Title, mailing address (even if current on record), and phone number
- Vehicle Certificate of Ownership (Title) Application; signed by all owners on current record and notarized, showing WA resident address and requested mailing address, WA driver license number(s), and phone numbers.
- If the current record shows a Legal owner, the Affidavit of Loss must be provided by the lender. The Replacement Title will be mailed only to the legal owner.
- Affidavit of Loss (Title); signed by all owners on current record and notarized.
- This form may also be used to include loss of current tab/or plates for replacement at same time if needed.
- $35.50 Money order or Cashier’s check for Replacement Title only (no plates, registration, or transfer of ownership) Any currently registered plate will remain in service.
- If new registration/or new standard plates are to be included:
- Add new tab registration/or plates to the letter of request.
- It is a good idea to scan and email the documents to our office before you mail them; that way we can review the documents to ensure they are correct, and give you a total in the case of also replacing plates or tabs.
